IEP Accommodation Implementation Log

An IEP accommodation tracker for case managers and general education teachers: log whether each accommodation was provided when it applied, and the implementation rate calculates for you. The files open in Microsoft Excel, have no macros and nothing to install, and are yours to reuse as often as you like.

What it does

  • Data Sheet: 10 accommodations x 8 opportunities (class period, test or day) with dropdown marks and automatic rates
  • Mark Y (implemented), N (not implemented) or NO (no opportunity); rates per accommodation and per column count only the times it applied
  • At target / Review flag for the team, with a review target you set (the rate and the minimum applicable opportunities)
  • Blank cells are never counted and percentages show a dash, never a misleading 0% or 100%, until there is something to count
  • Settings: edit your implementation codes and counting rules (up to 8 codes), with a setup check
  • Example tab with eight fictional accommodations across Science and ELA; the data sheet prints on one landscape US Letter page
