A SOAP notes template for speech-language pathologists in Google Sheets and Excel: log each session in one row and get a formatted SOAP note to copy into your EMR or IEP system, plus copy-ready progress lines. What it does

  • Progress Summary: pick a client and report period for each goal's sessions, first, latest and average accuracy, the last 6 months and copy-ready progress lines
  • Clients tab flags PROGRESS REPORT DUE, plans or IEPs ending soon and notes not yet entered in your EMR or IEP system
  • Goal & Phrase Bank with quick phrases for S, A and P, a goal bank and an ICD-10 quick reference
